The Care Management Department of Northwest Hospital & Medical Center is comprised of Registered Nurse (RN) Case Managers and Social Workers who help by coordinating your care from the time you enter the hospital until you go home.

Our Case Managers can answer questions you might have about your insurance or Medicare benefits. Our Social Workers offer support and counseling to you and your family, and can also help you arrange for ongoing care after hospitalization. They can provide resources about hiring household help, referrals to Assisted Living residences, as well as Skilled Nursing Units and Nursing Homes.
