Medical Plan - Summary of Benefits Coverage 2013
Group Legal Plan
Services are provided through Caldwell Legal, USA. The cost of the plan is $5.00 per month.
Waive Benefits
If you work a minimum of 24 hours per week (0.6 FTE) and certify that you have group health insurance from another source you may elect to waive your benefit coverage for a 15% wage premium. Please review the form carefully as certain restrictions apply. To waive benefits print and complete the 15% Wage Premium in Lieu of All Benefits form.
Employee Assistance Program
The Life Assistance & Work/Life Support Program can help you find solutions for everyday challenges of work and home as well as more serious issues involving emotional and physical well being.
Retirement Plan
If you would like to enroll in the plan, or make changes to your current investment options, or payroll deduction go to www.fidelity.com/atwork to access your account. You may also call Fidelity at (800) 343-0860.
